All Student-Athletes who are currently prescribed a medication by their Physician, that is banned by the NCAA, MUST provide all Medical Exemption Documentation.  Forms can be found in the links below.  Any further questions can be answered by the Saint Anselm College Sports Medicine Staff.

All first-year student-athletes must register at the NCAA Eligibility Center to be cleared for initial eligibility. Please remember to contact your school to forward your final grades and transcript to the Eligibility Center upon graduation. To determine the status of documents sent or to check on your eligibility status, contact the NCAA at (877) 262-1492.
